About Transitional Care Hospital
Originally established as a COVID-19 facility to manage patient overflow during the pandemic, Brackengate Field Hospital has since transitioned into a Transitional Care facility. Since June 1, 2023, it has fully implemented the Transitional Care Policy, offering a new service package with 128 beds. This unique model of care prioritizes wellness, aligning with the Western Cape Government Health’s strategy of proactive disease prevention and health promotion.
Providing Transitional Care Services
Transitional Care Hospital offers in-patient Transitional Care services, managed by a multidisciplinary team, tailored to the functional needs of each patient. As part of the Western Cape Department of Health’s strategy, Transitional Care contributes to the expansion of Transitional Care services, focusing on optimal recovery from acute illnesses or complications of long-term conditions. The facility aims to alleviate pressure on acute hospitals in the region while maximizing efficiency, equity, and quality of care.
The facility’s multidisciplinary approach enhances patient outcomes
Transitional Care features specialized wards including Medical, Palliative & Wound Care, Rehabilitation, and TB, with Rehabilitation comprising approximately half of the patient configuration. Patients typically stay between two and ten weeks, undergoing rehabilitation with family involvement throughout the treatment process. The facility’s multidisciplinary approach enhances patient outcomes, ensuring they leave stronger and better equipped for continued care at home or in the community.
Professionals at Transitional Care
Our team comprises Medical Officers led by a Specialist Family Physician, Nursing staff including Professional Nurses, Enrolled Nurses, and Enrolled Nursing Assistants overseen by a Nursing Operational Manager. We also have dedicated professionals such as Pharmacists, Physiotherapists, Occupational Therapists, Speech Therapists, a Dietician, Social Workers, Auxiliary Social Workers, a Radiographer, and Non-Clinical Administration and Support staff.
